My birth certificate registration no. COC/1975/HQ/055/001363/0 date of registration 20/11/1975.

Would like to do the following inclusion:
1. Name of child: Ramapriya Srinivasan
2. Father name to read as P.S.Srinivasan and not as P.A.Sreenivasan.

Could you plz tell which taluk office should I go and do the changes and let know the procedure and charges

Presently I live Mumbai, would like to authorise my father to do these on my behalf. Hope the following document would be sufficient :
1. My Passport copy
2. Authorisation letter
