My daughter was born at berhampur, dist -Ganjam ,orissa. date of birth may 2005 (Name-Bishnu) .Now Iam working in hyderabad since 10 yrs. during admission in school in L kg I change her name(Sunena) ,she studied up to class 6th. now I am changing her school to join in class 7th. Now the school is providing T.C as per changed name Sunena ( not giving as per birth certificate)
.In Adhaar card her name is recorded as per changed name( Sunena). We did not do any affidevit .
How to I change her name in birth certificate as sunena from bishnu.
Is only a affidavit in Notery is sufficient. or I have to change in municipality also.
If to change in municipality what documents they want.
