I have lost birth certificate of my Daughter in 2005 floods. Now

I have lost birth certificate of my Daughter in 2005 floods.
Now I need a fresh certificate for applying a passport in December 2016 when my Daughter turns 18.
I have SLC from school of class 12th which has mention of the date of birth.
How can I get a fresh birth certificate on the basis of this one document from the authorities.
