I need to rectify my name in my son's birth cirtificate. His all details are correct except my name. My maiden name was Kasturi Krishnatreya, but after marriage I am Kasturi Mahanta, but while filling the form to issuing my son's birth cirtificate it was writtren as Kasturi Krishnatreya Mahanta. But in all my identity cards are issued and using my name is appearing as Kasturi Mahanta only.So I want to rectify my name to Kasturi Mahanta in my son's birth cirtificate. Pls advice how can i proceed for the same
