My niece is presently staying in USA along with her mother. Her father has already expired. She needs her birth certificate containing her name. Her birth is already registered with New Delhi Municipal Council. Her name needs to be included in the birth record, beside correction in the name of parents as the record mentions only initial names and not the surname of parents. In her academic certificates, her name is Ashima Bhakri, while after marriage she is known as 'Ashima Shukla'.
Pl.inform how can she apply for inclusion of her name and addition of surname in the names of parents and what documents are required for the purpose? Which name should be included in birth certificate i.e. the one before marriage (Ashima Bhakri) or after marriage (Ashima Shukla) or both? Whether I can apply on behalf of my Niece? Thanks.
