Age Confirmation Form for New Pension Enrolment

On 14th Nov 2014, I visited Yelahanka Government Hospital to get a Age Confirmation Form (Proforma-2) for my mother. My recently expired Father, being a government servant, allowed my mother get pension. One of the requirements for pension application was this form.
The attender at reception immediately responded to our inquiry and intimated that it would cost 200 Rs. He collected the amount from me and took us to Dr. K.Jagadish, Senior Specialist.
He handed over the amount to the doctor and doctor prepared us the form. There was no receipt given.
