Dear Sir/Madam,
I was born on 4th Nov 1981 in 158 base hospital Siliguri West bengal which my father was serving the India Army. We not sure if my birth certificate was issued or not, as we can't find it. I have an urgent assignment for France where birth certificate is mandate, since last 4 days i am trying to get some information or contact info for the base hospital but in Vain.
Could you please help me?

Mother's Name: Susama Satpathy
