Hi ,
I need to get my name corrected in my child 's birth certificate . My first and last name are mentioned correctly in the certificate , but I need to add my maiden name as all my identity proofs have my complete name i.e name, maiden name , surname after marriage .My child is over a year old now . What is the procedure and what documents do I need to submit .
